 Background:

Provide outpatient and/or inpatient medical care to patients/beneficiaries according to adapted and updated medical knowledge, MSF protocols, values and universal hygiene standards to improve the patients/beneficiaries health conditions



 
 Duties and responsibilities:

  • Apply medical knowledge and skills to diagnose and prevention. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols. 
  • Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the treatment to follow, checking they have understood. 
  • Follow up the evolution of the hospitalized/IDP patients, through daily visits , consultations and examinations, prescribing the necessary treatment following MSF protocols, deciding whether they can be discharged or transferred to other departments – in collaboration with other doctors- and informing their family about the patient’s evolution. 
  • Check and control the rational distribution of medicines and equipment under his/her responsibility and take care the quality, disinfection and sterilization of the medical material. Assure general compliance with standing hygiene standards. 
  • Participate in the collection and analysis of epidemiological data, checking its validity and informing the line manager or project coordinator about any problem or complication of the patients illness, medical error and monitors the proper functioning of the department, equipment or material. 
  • Ensure ongoing training of the medical/paramedical multidisciplinary team in order to optimize the quality of care. 
  • Knows and ensures all MSF medical protocols are followed and implemented, checking universal precautions are followed at all times and reducing bio-hazard risks and improving infection control. Ensures professional confidentiality is respected. 
  • Manage the team under his/her direct responsibility according to MSF HR policies and procedures, supervising their performance, organizing and scheduling shifts and rotations, directly participating in emergencies and on calls if necessary.

In April 2023, civil war broke out in Sudan. Clashes have largely been focused in Darfur State, Khartoum State and Al-Jazeera state but it has extended beyond these areas. MSF OCA has maintained a consistent humanitarian presense in Khartoum State throughout the war, with activities largely being based in Al-Nao hospital. More recently, MSF has expanded activities in Omdurman to include interventions in Albuluk Pediatric Hospital. The NTS will take an active roll in planning and implementing all nursing activities within this project. 

 

The aim is to improve patient care in the emergency department(s) that MSF supports. This includes capacity building, quality improvement interventions, and other ways of improving outcomes in collaboration with the SMoH and hospital staffs. The doctor will work closely with the MAM to identify gaps and obstacles and figure out ways of solving them.

 

Specific Accountabilities in addition to the above;

  • In collaboration with the SMoH medical staff identify a way to work together and play an active role in case discussions and management
  • Be involved in SMoH medical meeting relevant for this department, to ensure MSF involvement and collaboration. 
  • Identify training needs for the medical doctors, in line with the desire of SMoH/hospital management, to improve the medical management in the ER, such as ATLS, ABCDE, cardiac resuscitation, technical skills such as reductions/repositioning, minor surgery, chest tubes and relevant pain management techniques (procedural analgesia and sedation, localregional nerve block, oral and intraveneus painmanagement).
  • Be closely involved in the department’s regular triage mechanism, ideally via the WHO format IITT (Interagency Integrated Triage Tool). Will design and conduct training for relevant staff (in collaboration with the NAM) in order to improve triage flow. 
  • Assess and monitor bed occupancy and average length of stay. Will identify factors that will improve these figures and propose a strategy to the line management (MAM) in order to implement the suggested strategy. 
  • Play an active role in the the Mass Casualty Incident Planing: participate in meetings, organise the MCI triage flow and management according to area. Conduct relevant training (simulations, table top exercises) and in accordance with line management go to sharepoint for relevant materials and get in touch with ER advisor in case additional support is required. 
  • Focus on rational use of antibiotics, in line with MoH strategy to increase antimicrobial stewardship. 
  • In case there are obstacles or barriers in the collaboration with SMoH, flag these to the MAM, so that a constructive solution can be found in order to maintain our good relationship.
  • Play an active role in the observation of the auxillary services: lab (including bloodbank), radiology and pharmacy. Observe what are obstacles that might interfere with smooth patient flow in the ER and in collaboration with NAM and line management, propose improvements and followup on their implementation. 
  • Work together with data encoder to ensure proper collection of data. 
  • Contribute to the medical narrative for the monthly medical report, in collaboration with the MAM
  • Discuss with the IPC supervisor and/or NAM whenever important hygiene matters are observed, that should be improved in order to contribute to positive patient outcomes. 
  • Actively participate in MSF medical and general meetings held in the MSF Holland Office. 
  • Complete the following courses on Tembo as part of their onboarding: antimicrobial stewardship, routine triage with IITT (6hrs), Mass Casuality planning (2hrs), Mass casualty triage (3 hrs), MSF Field Simulation scenarios: Mass Casualty Incident

 
 Minimum Qualification, Skills and Experience Required:

Education: Medical Doctor diploma in a recognized university.

 

Experience: 3 years’ experience minimum as a Medical Doctor in emergency clinical work (after medical training) essential.

 

Languages: English and Arabic are essential.

 

Knowledge: Essential computer literacy (word, excel and internet).
